If, while engaging in any club activity, event or training, any participant has an incident or accident please fill out the incident report form, no matter how small or trivial the incident may seem.

The form can be started by one person immediately and once submitted a receipt with a link to edit is sent to that person.  This link can then be shared with other club leaders, instructors and any other relevant persons who were involved in the incident or aftermath of it for their input to the report.
